INSTRUCTIONS
• Read the appropriate course textbook
• This is an open book test. Answer the questions then fill in the answer sheet.
• Please note: This is a GENERIC answer sheet. Your answer sheet may contain more numbers than the actual number of questions on your post test. Fill in the appropriate number of answers; leave the rest blank.
• A score of 75% or higher is needed to receive CE credit. You will have a maximum of three attempts to pass this course.
• Please verify that the COURSE NAME at the top of the answer sheet matches the POST TEST COURSE TITLE.
• You have one year to complete this course before it expires.

SUBMIT or print and FAX or MAIL the Answer Sheet

• The "SUBMIT" button at the bottom of the answer sheet will email your completed answer sheet to ScrubsCE. You can also print and mail or fax your completed answer sheet back to us. Make sure your personal information is correct. That information will go on your certificate.
• FAX to 480-393-5670 (during regular business hours of 9a.m. – 5:30p.m. CST Monday thru Friday). Emailed or Faxed answer sheets will have the same business day certificate issued via email upon successful completion of the post test. You will NOT receive a certificate by regular mail. If you would like a copy of your certificate by mail, we would be more than happy to send it to you upon your request.
• Internet transmission errors can occur. We are not responsible for internet or fax errors or failures of any kind. Please make sure the answer sheet did transmit.
• If you mail your answer sheet, make a copy before you mail it.

CERTIFICATE OF COMPLETION
• Successful completion means you have to receive a grade of 75% or higher. If you take a test three times and cannot achieve a passing score, you will not get CE credit for the course.
• The certificate will be generated the same business day.
• Keep the certificate in a safe place. It is your record of having successfully completed the course.
• If you need to show proof of your certificate, send a copy and keep the original.
• If you lose the certificate, we will be happy to provide a duplicate at no fee.
